    Vitals.com is hiring

Vitals.com currently has positions for PHP, Ruby/Rails, and/or AngularJS developers, as well as QA/Test specialists, both in our Lyndhurst NJ (near NYC) and Tulsa OK offices. If interested, PM me here, and I will provide you an email address to which you can send me your resume and cover letter.

Vitals specializes in the fast-growing health-care area, specifically in providing patient empowerment tools via the web, including products we create for various medical service providers and insurers.

I don't really know for sure. We do have one guy working from TN, though he moved here for about a year when he started. So I guess it's not impossible? Might require you to impress the decision-makers at least a bit more than any local applicants. Of course, if they can pay you MO wages instead of greater NYC wages, that might be in your favor.
