How is that some sites allow you to go the site and the url remains simply something like http://www.site.com/ without seeing the name of the php file running the site?

most probably they are using top frame, which loads everything in body frame, so you dont see the url you are at, and only the root domain is visible.

You could make an index.html file that is one big frame definition, that loads index.php3 or index2.html or so inside it.
