PHP Submit Form w/ Attachments?

I am looking for a way to design a signup form in PHP that will allow the person filling out the form to attach 4-5 imgaes to the form and the form once submitted store itself in a webpage format showing all the persons info as well as the pictures they submitted. The form would also send an alert email to the form owner alerting them that their is a new signup awaiting them.

Can someone help me figure out how this can be done in PHP please, I would prefer to store in flatfile but mysql would be ok to.

Use the post upload functions at:
http://www.php.net/manual/en/features.file-upload.php

They are quite easy and painless....
