FORMS and Image Errors

I have a FORM @ http://www.moconet.com/exterior_contractors_index.php

After selecting the information by using the 3 drop down boxes - the next PHP screen uses several external files, both Javascript and PHP (include()😉.

I have the weather posted for our area as well as other afiiliate banners. These images are pulled from another source other than my server. They will not display. Is there a way to get around this? I have tried just a basic FORM and and external image from another source and that doesn;t work either.
