Form to Database to HTML

Hey All, I am new at this and was wondering if some of you PHP geniuses could help me get some code going to take data from a form, put it all into a database (mySQL or flatfile) and then have an HTML template in which a row/column's info can be parsed like Column Name, and # 4 into NAME: _______

Help anybody?
