URGENT - Check Boxes - PHP

I have added checkboxes. Now I want to delete the record where the checkboxes have been checked. I have tried out some of the codes in this forum, but still being an amateur, am still unable to complete my task. Please give some specific help. I would be extremely grateful. Thanks
