php is very wide in scope, and it's not very likely that a single person will know every aspect. So it would help if you are more specific about the job to be done.

1. Patient Record Management
2. Insurance Record Management
3. Medical History Retrival
4. Web Interactive

for a small medical office. (1 doctor and 3 assistants)
