Hello, is it possible to use a php form with a predefined text field and the sent an email to a specific adres with some attachment. And it mus be possible to store the data of different types of subjects in a database.

What I want to do is the following: 1. I want to make a input form for information sending to people. 2. This information needs to be stored in a seperate database for future use. 3. It needs to be possible te sent an email to this person including one or more attachment, which will be predefined according the first selection you make on the form. 4. Startform : Request Info Ask a Question Send a fax(??) if possible.
