URL submission

I'm trying to figure out how to get my URL submission form to add information to my tables! Any guides out there, to help a rookie out?

My URL submission form has something like 6 fields -- I have 6 fields in my table -- I want to populate my table/fields with this information! And then pull it up later!

Any crash course code out there anywhere! I think I've written a book, but no luck thus far! (I'm a PHP rookie, doing everything the HARD way -- guess it's time to ask for help!
