Counter w/o same browser-sess increments

Hi.

I am writing a counter for my website in PHP. I would like to know what the technique is to stop php from incrementing the counter everytime some person reloads their window (which would not reflect bogus increments).

Same goes for a poll system i want on the website. if some person goes on voting again and again in the same browser session, how to stop that.

for counters, how can i use IP's ?? i understnad i can record IP's and then check against the list for already existing IP's.

but then if a user goes back to my website after 10 days from the same ip, shouldn't the count be incremented?? how to do this/
