Did you try reading the PHP documentation, any PHP books, or even perhaps download a free PHP program (a good list is available at php.resourceindex.com) and see how they do it?
This is very common and fairly basic PHP coding. If you've written some code and its not working, post it here and we'll be glad to comment on it or help you get past problems in it, but you should at least make an attempt first.
If your not interested in writing the code yourself, perhaps contracting a PHP programmer or commercial development firm is more appropriate for your needs?
