Hide Image Source, img src="image.php"

I saw another site use <img src="image.php"> and I could not discover the true source for the image that was displayed even after poking around. I need this capability. Any ideas on the contents of image.php AND the image source is located on a remote server.

all image.php might be doing is setting the headers to the file image type and sending the contents of a image file. the actual image file might be anywhere on the server, and may not even be accessible directly over the web.
