mail "from" problem

i used below code it send mail but i want to display "My Name" at From but instead it display something like "annonymous@helle....
what might be the problem please help me

mail("[e-mail]","$subject","Name : $Name\r\n Address :$Add\r\n Email : $Email\r\n Title : $Title\r\n","<My Name>");

[Anon]

must be "From: $name <$email>"
