i will start off by saying this is the first time i have ever used PHP.
What i have is a form that gets emailed out when submitted. I have radio buttons in this form. My problem is having the radio buttons show a yes or no answer in the email that gets received. My html code is now this

my corrsponding PHP code is this, it is the "do i want to preboard yes / no " that i am having problems with.

Another problem i have is that the end user filling out the form has to input the receivers email address into the form. I want the address to be their by default or even better, to be hidden . any ideas
