customer input/order request form

I'm looking for a script or an idea or something to track my customer job requests.
For instance, say they need some changes done on a website, or an email account added or a graphic changed. I want them to be able to submit their request, then have the request emailed to me, but let them be able to answer/or update my response, or close the order. Each job request should only be visible to the client who submitted it, with a listing of all jobs for that client. Perhaps accessing their "area" with a password.

Too tall an order?

If you have seen a script like this or know how i can modify an exisitng script, I'd appreciate all suggestions!

I don't know of any pre-existing code to do this (and I took a quick poke around) - but as a custom job could be done in a few hours.
