str_replace to format phone number

I am working on a script that takes in a phone number from a form.

I would like the users to be able to enter any of the following formats:
800-555-1212
800.555.1212
8005551212

i need to take any of those values and put them into the top format (800-555-1212) before they go in the database.

i'm not sure the best way to go about this.

i've used str_replace("-","",$phone) and str_replace(".","",$phone) , but i need to then put the dashes back in.

any suggestions would be greatly appreciated.
