I'm trying to call teh function ...

Every time I load the page ..I get ..
Fatal error: Call to undefined function: make_checkbox_html() in /export/home/sjanis/apdigitalsupport.mm.ap.org/htdocs/infoclient/betasurvey.php on line 5
