I am new to PHP/MySQL. Can anyone give me an example..maybe something small.. like a form that sends data to MySQL and then a another page that lets me do a search. Maybe like a phone book sample.

Give this tutorial a shot. It's a good primer to PHP and MySQL rolled into one and includes lots of useable sample code to get a budding PHP'er on their way. Note it's written with reference to PHP3 (written in 1999) but still does the trick.
