Converting ASP to PHP

I have tried the ASP2PHP program but get quite a few errors & cannot fix it.

I have a really small ASP file, if possible can somebody convert to PHP for me please?

Ahh I have made an error thats was for a SQL database, what do i need to change for a mySQl database?

Or do I not need to change anything?
