How Do I Attach A File To An Email?

I have a script that sends an email via an HTML form, but I would like to have the ability to attach a file to that email from the HTML form. Does anyone know how, or have a link to a decent script that could help me accomplish this?

I tried that one, and for some reason, the attachment shows up in the actual email as a bunch of garbage, not as an actual file attachment. Not sure if I am doing something wrong or something, but I can't get it to work... sigh

Something must be not working correctly, because it shows as garbage every time. I have tried messing with it for awhile now.

I have tried numerous scripts, and ALL the scripts return the attachment IN the message as garbled junk. None of them create actual attachments like they are supposed to. Has anyone had this problem? If so, please let me know what you did to fix this. I am stumped for sure. Thanks again...
