Need a Nerd to Develop 30 Web Forms

Hello,

I work for a community college. I have been asked to recommend an electronic forms developer to create about 30 forms.

We have the following requirements:

-well documented code for future inhouse tweaking

-create seperate web report interfaces for each form so secretaries and can push buttons and get good reports.

-please describe any potential validation or signature options

Please contact me with your initial proposal if you are interested. The closer you are to the Los Angeles area the better, but we are open to working with someone long distance if they can do the job in a timely and professional manner.

but serious.. its quite easy to make a class for creating forms on the fly.. did a lot of Content Management System building and did that trick more then once.
I guess there are even good classes on the Internet ready for download and use..

Hmm, I guess I missed including an important piece of information. The school is considering either a commercial or open source solution. I have argued that open source will be more cost effective, and tweakable (that's why I requested well documented code.)

Without seeing all 30 forms (something I need to have scanned) it will difficult for you to come up with a quote. I am figuring about $100 bucks per form or so. So the total project will be around $3,000, with documented coding, and a report / maintenance interface for each form. I think each form would also need a sperate MySQL database, but the whole thing could be off of one server.

I hope this clarifies the situation.
