Flat DB survey script?

I'm trying to find a decent flat-DB script to build a simple survey in order to collect some opinions from unregistered users. Does anyone know any?

There are some good polls and votes scripts but haven't seen a real survey with multiple answers and text fields.

Use any MySQL script with those functions (many at hotscripts ), and use DBX along with it. simple enough ... eh 😉

Yah, i took a look at the dbx. Looks interesting, might be studying that later on more deeply.
