I am setting up an online survey for my client and need some directions to start with. I am going to use php/mysql to get
this task done.
1)The survey is targetted to about 800 of my client's customer. He thinks of giving them an access codes say from 2000 to 2800 and ask them to fill the questionaire. Is this a right way to go or any other better approach?
My concern is what if a customer fat fingers do you think I should
associate the number with a password. Any direction/ideas for this using php/mysql very much appreciated.
2)Collecting the responses in the DB as well as storing in an alias
mailbox as a back up. I have not seen/designed a code which will both submit and send as an email any form. Is it possible or if you any ideas/direction again thanks a bunch.
3)Tabulation of results:

For the questions with a 1 through 5 answer options. 1 represents 20, 2=40, 3=60 4=80 5= 100 to be tabulated as points The tabulation should just total up all the scores and divide by the number of answers. For the NA if the program could just tell us the percent and number of NA responses.
On the yes and no questions, if we could just get a percent and number of yes and percent and number of no response.
On the offer you own comment types, we could just have those pasted to a seperate page to be printed off that would work fine. Otherwise, if the fill in the blank questions present a problem, we can drop those.

I am also thinking about using GD libraries to generate the graphs. Pls pour me with some inputs/thoughts and directions.
