parsing info from a .csv table

HI guys,

I am writting an app for a logistics operation where they need to keep track of some items over various locations and shipping and recieving docks.

The scanners that are being used will save the data to a .csv file with various headers. The headers needed need to tell who is actually scanning the item, the location of this person.

for example (Items enclosed in [] are not actually included just shown for convienience

Scanning Point
[person scanning] John Davis

Recieving Dock

I want them to be able to upload this file with the program that came with the scanner. This scanner will, at the time of uploading, put all of the data in one file, as shown above. Basically I need the php file to read the data and put it into a mysql database. For example it would put the data from the scanning point into the mysql database as

123-000-089, 4 ,John Davis
123-000-955, 4 ,John Davis
123-050-654, 4 ,John Davis

After doint this it will insert the items recieved into the table holding information on recieved items.

Any Suggestions?

I hate documenting code so just ask me if you dont understand anything in here but please check the manual first if its a function.
