I have a search results form from a MySql database query and at the end is the option to add a new item.

The question is:

How can I display a pop up window with the input form in it which when submitted will close the window and refresh the search results? (or am I asking the impossible?)

then you will need javascript,
the function window.open() open up a window
with window.close() , close the window
