mysql/php form builder?

Can anyone recommend a php based forms application that creates a web front end to search multiple Mysql tables. The ones I have seen so far that look fairly good are DaDaBIK and the excellent Appbuilder suite from Jay P Narain which includes the Form Wizard (although I am struggling to get even the example working)

Try out NuSphere's PHPed.

It has a cool form building plug in.
