dynamic forms using mysql

I need to setup a database that holds football match fixtures/results, players and scorers.
I have fixtures and results as one table, players as another and another table which holds performance statistics such as who played in what game and how many goals they scored.

The trouble comes when i need to enter the data into these tables using a form. The form must be dynamic. So when a person enters a new match, the form gets a list of players from the players table and displays them with a check box next to them. The check boxes allow the person to select which players played in a certain match. When x no of players have been selected how can i then put them into the table on separate rows. They must be on separate rows as other information on the row could be things such as goals scored etc.

ooo I like these problems.
You must:
1.) Generate a list of all the apropriate items. (players in this case).
2.) Spit the players out as checkboxes.

name="players[]" value=$curplayer

3.) Look at the array on the next page to get the players selected.

To build the form table, you need one loop to spit out rows, and the an inner loop to spit out columns.

Let's say you want the first column to be the checkboxes.
