Need help FAST!

Earlier this year I hired a developer to do some work for me, all was well the job was done and we parted. I would like to make some additions to the job he did but he is no longer with the co. and I can not get a hold of him. So I broke down and tried to figure a few things out for myself. I got it almost all working except for a few bugs.

He wrote a program where a user can enter data into a form and keep a listing. I am trying to make it so they can go through a list of items and select things to be added to their "personal database". For some reason parts are cut off or not added at all. I am pulling my hair out here and desparately need help figuring this out. Is there any kind soul out their to tell me what I am doing wrong here????

Anyway, the app that he made is so users can keep track of a list of coupons that they use. They can add the coupons and keep track of the value, exdate and quantity to make creating their shopping list a little easier. Anyway some people trade coupons with other people so they can go in and select how many they are sending that person and it automatically adds those coupons to the recipients database. It takes forever to go through and enter long lists of these coupons, so I want them to be able to go through a list of coupons and select the ones they would like to add to their database.
