I think in your php.ini register_globals is set to OFF,
so you have to use $_SERVER[PHP_SELF] instead of $PHP_SELF.

At the beginning of the script you could code:

if (!isset($PHP_SELF)) {
   $PHP_SELF = $_SERVER[PHP_SELF];
}
