PHP/MySQL/Forms Variable Problem

Hi,

I seem to have this variable problem in my script. When I upload the file and insert it into the database, it doesn't seem to store the username and description of the file that I get from the form. It's really frustrating and I really don't know what to do.

Here is the code. Any quick help would be really appreciated.

}
