Form completed by users -> Act! or a PIM?

Right now, users fill out a form indicating interest in the site's services. The form is then sent via email.

What I would like to do is have users fill out a form and then have the input formatted for Act ! Contact Management software (it's a sales tool with organization of contacts) or some other PIM app (other than Outlook preferably).

Has someone seen a script around doing something like this?
