optimum performance

hey guys, i was about to make a table for user qualifications, and i've got 2 options. the first field is going to be their id, and then i can either make 5 fields for each qualification they supply at registration(they can have up to 5 qualifications). but here's the catch, each qualification is either verified or not(1 or 0). so should i make it like this:

ID | Q1 | Q2 | Q3 | Q4 | Q5 | Q1V | Q2V | Q3V | Q4V | Q5V

Q1 being the qualification, Q1V being whether Q1 is verified or not

or should i make a seperate entry for each qualification like this:

ID | Qualification | Verified

My reasoning?

Number 1 result would return 1 result row

Number 2 would return 5 result rows.

I'll let you work out which query would give optimum performance.
