looking for a script or tutorial re: zipcodes

I need a php script or tutorial that shows how to calculate distance from:

To be based on zipcode entered in.
I have found a zip code table with long. and lat. but am wondering if anyone has, or knows of, a working script / tutorial on how to have a form element that someone can enter in their zipcode, and then it shows entries in a database that are close to that zipcode.
