upload image and data at same time? Can it be done?

Am attempting to create a page where members can upload a picture and some info at the same time.

The pre-requisite is the the next page show the photo and the data together in finished format.

Do not desire to insert the photo in the database. Would prefer to drop them into a folder and refer them by name. Maybe a combination of user attributes (fullname + last 4 digits of phone number)?

Am still fairly new to this and am relying on examples to learn better how this is done.

Should I apply a few variables to the filename to keep them unique?

Should I (try to) depend on the user properly naming the files?

Should I have a temp file location until they are approved? aka check box in admin panel for the dbase?

Make a form with the information you seek and a file input box for the image. Have your next script (or segment of code) rename the input file before copying it over to guarantee a unique name. PHP has a function for this (I'm sorry, I forgot what it is called) but I typically prefix the filename with a date and time stamp.

Copy the file over to its new location. Then, insert the information and the file name together into the database. This way you can do a simple select to retrieve both the picture and the accompanying information.

This is just a brief outline. If you need help with specific code segments, ask. For instance PHP can check and verify that the uploaded file is an image.
