Templates for SQL and PHP

I'm looking to design or looking for templates regarding how to create a database such as follows:

1. Allows the ability to import data from an excel spreadsheet consisting information such as:

Name
Address
State
Phone Number
Info

2. Once data has been imported, then have the ability to view all data.

3. Have the ability to sort fields by: Name then Address then State or whatever. But have the ability to sort not just ONE column.

Anyone know or can help me develop this??

Anyone can help me. I'm just starting out and want to at least see a simple database usage such as stated above. Once I can get this started it would hopefully help me extend the simple basic capabilities above.
