Form input into a server based txt file

Please Help. I am completely new to PHP and despite reading nearly all the articles in the forum, and every page in the 3 PHP books i.ve bought. I just can't find the correct process for transferring data that has been entered in a user form directly into a log.txt file that I could access. All the tutts I've seen go into using email and/or MySql where as all I want to do is a simple form with name,email address,comments and have the submit command transfer the user inputs into a txt file on the server and maybe return an nice thank you reply.

I know these procedures are meant to be simple but I just keep going around and around these forums in a vein attempt trying to find what I need ..... Please help before madness takes over.

you may need to modify read/write permissions within the directory that you want to save the file to.
