feedback required Please!!!

Hi I have being trying out PHP for a couple of days, and so far I have downloaded and installed php4 and apache got that working. Bought 3 teach yourself PHP books including the "sams php in 24 hours," and spent many hours in these forums, and at last have created a user input form which takes the users input and pipes it out to a server based text file, can anybody please take a look at my form and tell me if I have taken the correct approach I would welcome your feedback both positive and or negative.

Looks fine to me but then I can't see what you're doing with the PHP bit after submitting the form!

Thanks for your comments; unfortunately as I am at work I haven't got the php code to hand. However as you said it looks ok.
My next move will be to add some code to validate the submitted email address and check for any bad language and then after the thank you screen has been shown set it to auto return to the page before the form! well that’s the idea!!!
