This is very simple. You need to log into your phpadmin site.
From there your main screen should show table name and field.
Create a table name.
Enter in how many fields this table will contain.
Once entered, it will bring you within the table showing you the number of rows that you have entered and then from there, populate it with the fields you are going to database.
