quote request link from resultset

I would like to give users the ability to request a quote, from a link against individual items in a set of product results from a mysql database. The quote request only needs to link to a form with some of the details from the resultset. So, if the user likes the look of the Blah chair, for instance, they hit the link to a form with the chairs details already filled in. Can't think of the simplest/best way of doing this. In this particular case, I prefer to give individual quotes - the reason for the form.

Thanks a lot for posting this. I am not ignoring it...I just have not had the time to work with it yet. Much appreciated all the same.
