Can anyone point me in the right direction where i can find an email form that sends the "sender" an autoreply.
But it also must be possible to include(or allready available) that option thing ..like this >

I have found a few but they messed up when i included this.
or maybe some of you readers made one yourselve perhaps ?
Big problem is to find the one that i need,so some info :
My form needs a total of 6 fiels
Name ,
email ,
message ,
day(a date they have to fill in) ,
time(time they have to fill in) ,
and that option thing(or pull down whatever?)
