Hi all, I am in need of help setting up an sql database and php forms to collect and make available case studies of people suffering from a rare blood disorder.
The problem is the disorder has left me with a short term memory problem that has stopped my capacity to learn new technology.
I read and read the instructions and cannot make sense of them. When I do get to make a page or get something to work, I have forgotten how I did it by the next day.
What I need is someone who can make a basic database and a form to get information online from fellow sufferers, and another form to make it visible online for doctors and others interested to see.
