Take a look at this line:

header("Location: <a href="http//www.beitickets.com/index.php?login_error=1" target="_blank">[url]http://www.beitickets.com/index.php?login_error=1[/url]</a>");

//IT SHOULD BE:
header("Location: [url]http//www.beitickets.com/index.php?login_error=1[/url]");

You do not need to use the &lt a href tags inside of header: location

Look at line 4.

You didn't use a backslash before the quotes that were suposed to go into the output. It gets confused from that point forward.

Here's the wrong way:
$text-with-quotes="He said "Boo"!";

Here's the correct way:
$text-with-quotes="He said \"Boo\"!";
