I have been requested by a manager to build a PHP based form that allows users to fill in the blanks and then have it mail to them.

I dont know a thing about PHP.

We need the following -
Name, e-mail, Phone, Subject, and comments

The subject must be a drop-down list and needs to change the $recip based on the choice. I havent firgured out how to make this work.

IE
Option1 goes to Manager1
Option2 goes to Manager2
Option3 goes to Manager2

Can anyone help me.

This is a VERY BASIC and UNTESTED script without any features
