Does anybody what this means exactly,

If you purchase the script, we can set up a default version of the script.
But setting custom fields, categories other options will be your work.

it basically means if you buy it as it currently is without any modification it will cose (whatever it is).
if you want some modifications to the script, like another field, category, etc... it will cost you extra.

basically they would be chargning your for coding the changes to the current script.
