Page executes twice - Randomly...

I'm stumped on an MSSQL / PHP issue and I need some help.

Has anyone EVER seen a php page execute twice at random times? I've got a fairly large business application that runs mssql querys all the time. Up until fairly recently, we'd never had a problem with it, but lately it's been adding duplicate records occasionally. It seems to be without rhyme or reason.

I'm just curious if anyone has ever seen anything like this, and if so, what they did.

Here's what I've done...

Change all my calls from ODBC to MSSQL thinking that perhaps it's an ODBC bug (God knows there are enough of those).

Change all my include() calls to include_once()

Re-written a TON of my code thinking that perhaps some inefficiency was causing PHP to repost....

Any help would be appreciated.

Most of the time I see this it is someone hitting the refresh button. Just insert all records with a unique key that is generated each time an insert page comes up, so that any attempt to refresh will result in an error.

I wish it was as simple as stopping a refresh. The page executes almost instantly, and I know my business users aren't refreshing. 🙁

Is there any javascript or anything that could cause it like that? Either way, including a unique id that gets inserted into a unique field will usually stop this.

Nothing like that. To make things even more complicated....

There IS a Primary key on the table.

I tried this...

 
if($inserted = mssql_query($insert_query))
{
     echo "Page executed once.<br>";
}

after about 30 submissions of the form, not one time did I see that echo executed twice, but several times it duplicated records...

Go figure.
