PHP Upload script giving me a headache!

I have a file upload form located here: GO HERE

Here is my issue. The image upload works fine. A image will upload to my server, but all the data elemnst in the form are lost in the wind. Where do they go?? I dont know.

How would I:

#1. Have the image file upload to the server (as it is now) and then have all the data elemnts emailed to me.

or

#2. have everything in the form uploaded to the server all together.

I am very new at PHP so please speak in lamens terms. I have a feeling you will say I need to use - mail()

I have no idea how or where to place it within the script. yes, I am sorry, I am that new!😕

You should be getting the file using $FILES[] and the information via $POST[]. Are you? If not, that's probably what's screwing you up.

do you use this in your <form> section:

<form action="<?php print $PHP_SELF.'?do=upload'?>" method="post" ENCTYPE="multipart/form-data">

I mean the ENCTYPE="multipart/form-data" part. Try it and I guess you'll have your problem fixed.
