Help with a form

I am basically trying to add 8 fields to a DB table with 3 form fields. The first selection will somehow determine 5 fields and 2 others. Here is the form I got

Well, I already have a database of data. Concerts and setlist etc (http://www.hipbase.com/search.php). I want to create a bootleg list manager, basically they pick the shows the have and it is added to there list.

This is the form that will be used to submit the data, I want to use the info in my DB already to fill in the other 4 fields (Artist, Setlist, Location and Venue). The form (http://www.hipbase.com/mrlayance/requestadmin.php) is basic right now but the date is currently taken from the DB.
