survey/poll script

Can anyone recommend a good survey/poll script. The script must be able to incorporate images in the survey.

Just new to the forum so apologies if I'm posting in the wrong area.

what do you mean by incorperate images? like vote on an image?

Yeah

Basically I want the users to vote on a number of images (each image can will have a value of say 1 to 5, 1=bad , 5= excellent). That sort of thing.
