Opinion Lab Program?

Hello I was wondering if anyone knows of a (preferably free) PHP program like OpinionLab.com.

If you dont know what Im talking about check out hotels.com and look in the lower right corner.

I thought it was very cool but something I would rather write myself then pay for...

That is probably written in javascript. I'd check out the javascript websites like http://www.dynamicdrive.com.

They have a lot of menus and stuff there that you could modify to the same effect.
