Well ok....this is the thing....i play a game called outwar i'm sure some of u know what it is....well i'm setting up a crew website and i want to make a form that allows people to fill it out, i get an email and i see the forms info they filled out and i can invite them to the crew, however i want to use the php form codes not that old mailto: html code.
i've found a few scripts on the net and i'm totally illiterate to php language, so they really don't help me at all.....If anyone can help me out please either reply to this or contact me in the following ways

if u view the source, u can see the forms code....it looks simple enough, but i really dont have a single clue as to what to do to get a form like that on my site so i can have a setup like that for my crew.
my site is fully php compatible....it's php nuke 6.6 and i've got a lot of options...i'm capable of having this feature..
